ACCESS: Year-Round Enrollment Form  Referral Flow Chart

For Referring Districts: All supporting documents required to be submitted with this referral form must be uploaded to the ACCESS Referral secure file portal, which includes documents listed below for General Education and Special Education Students:​

​Students with IEPs: For students with an Individualized Education Plan (IEP), the school district must attach the required Special Education documents listed above in addition to the documents required of all students. ACCESS Special Education will consult with district representatives to ensure students are able to receive FAPE in ACCESS. If a student is in the process of being assessed for Special Education, please refer after your assessment and IEP is completed to discuss if ACCESS would be an appropriate FAPE. **Expulsions would be an exception to the above.** For additional information, please click the link HERE​.

We recognize that there may be situations where districts are unable to complete the online referral form. For students in the following categories, a print version referral form is available

• Currently in ACCESS
• Currently Incarcerated
• Foster Youth
• Unaccompanied Minor
