​Enrollment Instructions

  • Please read the following before completing the enrollment packet.
  • PLEASE PRINT THIS PAGE for future reference and recording appointment date and time.*

    1. A student must be referred to the ACCESS Program by the student’s district of residence.
      - The district of residence will submit a “Referral” form and “Course Request” form to the ACCESS Program. This will set in motion the enrollment process.

    2. The ACCESS Program will contact the parent to schedule an enrollment appointment.

    3. Record appointment information here:

    4. Complete the enrollment packet prior to the enrollment appointment. (click here for Enrollment Packet).
      - Do not sign or date forms, this must be done at the enrollment appointment.

    5. A Parent and the student enrolling must attend the enrollment appointment.

    6. Bring Enrollment Packet with you to the enrollment appointment.

    7. Enrollment space is on a first-come, first-serve basis.
      * Do not miss or be late for your enrollment appointment and risk losing the opportunity to enroll.
