The Title I Transition Program provides support for students to ensure a smooth and successful transition back to the community and their next school placement. Transition Specialists work with individual students to develop an educational and career plan. Incorporating collaboration with parents, teachers, and probation st​aff, an effective transition plan includes a review of transcripts, assessment of student needs, and monitoring of student progress. In addition, Transition Specialists offer classroom presentations on a variety of topics such as college, financial aid, employment skills, and money management.
